Bring a filled out Intake Booklet to your appointment. Intake Booklets are available to pick up at the Ingram Senior Center in Salem, NH. Intake Booklets can be dowloaded from here. If you have not picked up and filled out an Intake Booklet prior to your appointment, please arrive 15 minutes before your appointment to do so.
If you cannot make an appointment, please call 211 to cancel or leave a message at 601-647-6168.
The site will close whenever the Salem School District declares a snow day and is listed as closed on WMUR television. Site staff will call clients and reschedule appointments if a storm or other event prevents opening.
