Bring a filled out Intake Booklet to your appointment. Intake Booklets are available to pick up at the Whole Village Family Resource Center – Receptionist Counter. Intake Booklets can be dowloaded from here. If you have not picked up and filled out an Intake Booklet prior to your appointment, please arrive 15 minutes before your appointment to do so.
VITA program is generally for those who make $69,000 or less. Must bring paper copies of all tax documents.
The site will close whenever the Plymouth Regional School District declares a snow day and is listed as closed on WMUR television. Site staff will call clients and reschedule appointments if a storm or other event prevents opening.
