The following are needed to properly prepare your tax return:

• Filled out Intake Booklet. Many sites have Intake Booklets available for pickup prior to your appointment. Check our site listing for details. Intake Booklets can be downloaded from here for VITA and TCE sites or from here for Tax-Aide sites. . Please arrive at least 15 minutes early if you plan to complete the booklet on-site the day of your appointment.
• Government-issued picture ID such as a valid driver’s license for each taxpayer.
• Proof of Social Security or ITIN numbers for all persons on your tax return.
• Bank account routing and account numbers for direct deposit or debit. Bring a voided check, not a deposit ticket.
• Tax information such as Forms W-2, 1099, 1098, 1095.
• Last year’s tax return.
• All taxpayers must sign the return before it can be e-filed.
